National Disability Insurance Scheme (NDIS) Support Coordination

NDIS Support Coordination is a capacity building support that helps participants of the NDIS to understand and use their NDIS plan to pursue their goals. NDIS Support Coordinators connect participants with NDIS providers, community, mainstream and other government services. NDIS Support Coordinator build participants confidence and skills to use and coordinate their funded supports in their NDIS plans. Specialist Support Coordination is expected to address complex barriers impacting a participant’s ability to implement their plan and access appropriate supports. Specialist Support Coordinators assist participants to reduce complexity in their support environment, and overcome barriers to connecting with broader systems of supports as well as funded supports. If you have Support Coordination funded in your plan, contact your Local Area Coodinator (LAC) or NDIS representative and ask for a referral to Junction.

Junction is a registered NDIS provider of Support Coordination, including Level 3 Specialist Support Coordination. Support Coordination is a capacity building support designed to help NDIS participants understand and use their NDIS plan to pursue their goals. Our Support Coordinators assist participants to:

    • understand their NDIS plan and funded supports.
    • connect with NDIS providers, community, mainstream and other government services.
    • build confidence and skills to manage and coordinate supports independently.

Level 3 – Specialist Support Coordination

Junction provides Specialist Support Coordination (Level 3) for participants who experience complex needs or significant barriers that impact their ability to implement their NDIS plan. Specialist Support Coordinators work alongside participants and their support networks to:

    • address complex challenges within the support environment
    • reduce barriers to accessing services and broader systems of support
