Children’s Case Manager – Shepparton

In this vital role, you will manage cases at multiple support levels, from initial assessment to enhanced case management, ensuring that each child receives tailored support. Your responsibilities will include conducting comprehensive assessments, developing and monitoring flexible case plans, and facilitating therapeutic group work. You will collaborate closely with referring agencies and external services to provide holistic support, advocate for the child’s best interests, and enhance family well-being. This role involves working directly with children, assisting with their transition, and making referrals to specialized services when necessary. Your efforts will contribute to decreasing trauma impacts, improving family support, and fostering inter-service collaboration.
